Case Management

Duties: - Provide case management services to individuals and families in need - Conduct assessments to determine client needs and develop appropriate service plans - Coordinate and monitor the delivery of services to clients - Supervise and support clients in achieving their goals - Utilize motivational interviewing techniques to engage and empower clients - Collaborate with community resources and agencies to ensure comprehensive support for clients - Advocate for clients' rights and access to services - Maintain accurate and up-to-date case records Experience: - Previous experience in child welfare, childcare, workforce development, or related fields is preferred - Knowledge of Medicare, Section 8, and other government assistance programs is a plus - Strong communication and interpersonal skills - Ability to work effectively with diverse populations - Excellent organizational and time management skills - Ability to work independently and as part of a team - Experience in mentoring or coaching individuals is beneficial We offer competitive compensation and benefits packages.
