The Location:
Hale Makua Health Services Home Health is a Medicare-certified provider of personalized care in the comfort of your home for individuals who are home-bound and need skilled help with medical conditions.
The Position:
We're looking for passionate RN Case Managers to plan, develop, implement and evaluate Home Health patient's care from admissions to discharge, focusing on quality outcomes.
Admits patients to the agency and coordinates ongoing services for the interdisciplinary team under the established plan of treatment.
Utilizes the case management model in directing and coordinating patient care, ensuring quality care for patients of all ages, neonate to geriatric.
Performs re-evaluation, discharge, and routine nursing and aide supervisory visits as needed.
Requirements:
Graduate of an approved school of professional nursing
Current Registered Nursing License in the State of Hawaii.
Two (2) years' nursing experience, preferred.
Prior work history in acute, SNF or home health desirable.
BLS certification.
Current and valid 2 Step TB clearance and health review (can be done in-house).
