The selected candidate must hold a Hawaii LCSW license or have either LSW or MSW and working towards a Hawaii LCSW degree.
For the latter LSW/MSW, our health center will sponsor one-hour supervision per week with a licensed LCSW and commit to a two-year full-time employment agreement following obtaining their Hawaii LCSW license.
Additionally, if the selected candidate does not have LSW by date of hire, the candidate must obtain LSW within 90 days of hire.
$10,000 sign-on bonus for licensed LCSW.
PRIMARY FUNCTION: The Licensed Clinical Social Worker delivers consultation and co-management of medical conditions treated by our Adult Medicine, OB/GYN, and Pediatric Departments in a team-based and trauma-informed setting in our primary care clinic located in Wailuku.
This position provides clinical interventions to treat the psychosocial and behavioral aspects of chronic and acute illnesses.
This position applies behavioral principles to address maladaptive lifestyle and health behaviors within a primary care setting in accordance with the applicable professional, ethical, and legal standards.
Additionally, this position conducts intake assessments, individual, and group psychotherapy for mental health and substance use disorders, as well as for patients with dual diagnoses.
ESSENTIAL DUTIES AND FUNCTIONS Assessment skills: Recognition and treatment of psychological components of health problems, mental disorders, and psychosocial problems.
Early detection of “at risk” patients, aimed at preventing further physical or psychological deterioration.
Use of assessment questions and measures to assess current functioning, while also attending the psychological, behavioral, and physical components of health and wellness.
Knowledge of affective and cognitive components of health and illness (e.
g.
health belief models, medical problems that present as affective diagnosis, affective factors that influence reactions to diagnosis).
Knowledge of the effects of socio-cultural and socioeconomic factors of health and illness.
Treatment skills: Skilled in providing behavioral health consultations as well as brief psychotherapy.
Helping patient to develop cognitive and behavioral skills for patient management, and prevention and management of addiction to pain medications.
Effective and efficient management of patients with chronic health and/or emotional issues; assisting patient to develop self-management skills and provide education to assist patient with being active participant in their health care.
Assisting patients to cope with chronic health conditions, progressive conditions, and end of life issues.
Assist patients in adhering with medical treatment initiated by primary care provider, such as developing strategies to cope with medication side effects.
Focuses patient interventions and recommendations on functional outcomes and symptom reduction.
Proficient use of current evidence-based interventions to treat health and mental health issues, such as use of self-care, deep breathing, cognitive restructuring, sleep hygiene, problem solving, assertive communication, and motivational interviewing.
Conducts an evidence-based suicide risk assessment for all patients with depressed mood.
Work collaboratively with primary care physicians and other health care professionals to provide the best treatment and achieve desired health outcomes.
Other Duties: Attends and participates in meetings and committees as necessary.
Participates with peer reviews.
Attends additional training sessions as suggested and approved by the Integrated Health Director to improve deficiencies or enhance knowledge and skills.
Performs other duties deemed appropriate and at times selected by the Integrated Health Director.
QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential job function assigned satisfactorily.

Education and Experience Licensed Clinical Social Worker (LCSW), Hawaii Licensure or license eligibility is required.
At least two years of experience in primary care or other medical settings for LCSW license.
LSW/MSW - the health center will provide LCSW supervision weekly for MSW working toward LSW within first 3 months of employment and commitment to a 2-year employment agreement Education and training in health psychology.
Experience, training, and certification in addiction treatment is a plus.
Required Skills/Abilities: Competent to treat patients across the lifespan.
Competent in behavioral medicine and brief psychotherapy.
Ability to work effectively on primary care teams providing behavioral health consultations, care coordination, and consultations to the medical and dental staff.
Ability to deliver short-term, evidence-based interventions.
Effective use of screening and outcome tools.
Advanced interpersonal skills to promote and participate in team-based care, patient-centered medical home (PCMH) model, trauma-informed care, and SAMHSA’s levels of healthcare integration.
Knowledge of basic physiology, psychopharmacology, and medical terminology.
Competency in cultural awareness of the populations we serve and works from a frame of cultural humility.
Adaptive to the opportunities and challenges of living and working on an island.
Collaborate and be an effective team member.
Ability to multi-task, manage competing demands well, and complete clinical charts and documentation promptly.
Maintain ethical and professional standards.
Ability to effectively represent CCM in the community and maintain effective working relationships among co-workers, public, private and professional groups.

Access to an insured vehicle.
TB clearance within the previous 12 months.
